Like in Winter Soldier, when they mention Hydra keeping tabs on supers they mention Stephen Strange - waaay before he was Dr Strange. And in Dr Strange, they mention a possible surgical case for him to take on involving a soldier with below the waist paralysis inflicted by a flying suit of armour accident - which hadn’t happened yet in Civil War.
Plus Stephen Strange is a consultant and they wouldn’t call him Doctor.
