It comes in after a certain point in the story, so if you've got a late game save, you could get right to it. There's also an option to just play the DLC using a fresh character that starts at a highish level. It's still an improvement, of course, but the AI and logic still isn't quite right throughout the game, or can't match the game's ambitions. It'll always be a flawed gem.
Edit: Then again, I'd say the same about Witcher 3. It's combat and systems are pretty shonky. Maybe one day CDPR will figure that stuff out as well.
